Appointments:

New patients are seen by physician referral only, regardless of the type of insurance.

If you wish to be seen as a new patient in our office, please contact your existing physician and have them call our office with the referral. The number to call is 409-835-9834  ext 3527

If you are an existing patient needing to book an appointment please call 409-835-9834. Push “0” to speak with a scheduler.

If you are a new patient please be sure to bring the following with you to your visit:

 

  •  Medications or a list of medications with strength and dosage as well as the name of the physician that prescribed them.

  • Diabetics- Blood Glucose Monitor and a written log of your last 5 days of blood sugar results along with any other equipment you may use. (such as continuous glucose monitor, insulin pump)

  • Any pertinent lab or other information

  • A list of your past medical history

  • Your legal Identification (IE: Drivers License)

  • Your current insurance cards

NEW PATIENT WITH A SCHEDULED APPOINTMENT PLEASE ARRIVE AT LEAST 15 MINUTES EARLY TO FILL OUT PAPERWORK AND PRESENT YOUR ID AND INSURANCE CARDS.

Follow Up Appointments:

  • Have lab work done 1 to 2 weeks before appointment

  • Bring your blood sugar monitor and/or your continuous glucose monitor with you

  • to the lab as well as your office visit

  • Bring any new medications with you

  • Be ready to inform us of any illness, hospitalizations, procedures, or surgeries done since your last visit.

Follow up Appts

Diabetic Patients

  • Bring your blood sugar meter AND a written log of your blood sugar readings for the last 5 to 7 days OR

  • Bring your continuous glucose monitor for download and review

  • If you can, we advise you to download and print out your blood sugar readings and/or download and print out your pump information prior to coming in

  • If applicable bring copy of any testing or procedures performed since last visit

We accept most private insurance plans as well as Medicare and most Medicare Replacement Plans. For further information regarding insurance or payments please call

Notice

If you have an insurance that requires a referral it is the responsibility of the patient to obtain the referral prior to the visit. If the referral is not obtained the appointment is subject to be rescheduled.
